Chiro Care LLC, doing business as Chiro Care Clinic (“Chiro Care Clinic,” “we,” “us,” or “our”), is committed to protecting your privacy. This Privacy Policy explains what personal information we collect through our website at www.chirocareclinic.com (“Website”), how we use it, whether and how it is disclosed, and how we handle telephone numbers and SMS/text messaging consent.

1. Information We Collect

We may collect the following categories of personal information when you visit our Website, submit a form, schedule an appointment, or otherwise interact with Chiro Care Clinic:

  • Contact Information: Name, mailing address, email address, telephone number.
  • Appointment Information: Requested appointment dates/times, reason for visit, insurance carrier and general insurance-verification details.
  • Communications Data: Records of communications with us, including phone calls, emails, form submissions, and text messages (SMS).
  • SMS Consent Records: If you opt in to receive text messages, we record the date, time, the specific form or method used, and the exact consent language displayed to you at the time of opt-in.
  • Website Usage Data: IP address, browser type, device information, and pages visited, collected automatically through cookies, scheduling widgets, and analytics tools (see Section 5).


We intentionally limit what our standard website forms request. See Section 6 for information on sensitive medical information.

2. How We Use Your Information

  • Schedule, confirm, reschedule, or remind you of appointments.
  • Verify insurance eligibility and coordinate insurance-related communication.
  • Respond to inquiries submitted through our Website, including Contact Us, appointment-request, insurance-verification, and new-patient inquiry forms.
  • Provide customer care and general patient communication.
  • Operate and improve our Website and scheduling systems.
  • Comply with legal, regulatory, and insurance-related obligations.
  • With your separate, specific consent, send promotional or marketing messages about our services or offers.

3. Telephone Numbers, SMS Consent & Mobile Opt-In Information

If you provide your telephone number and check the optional SMS-consent checkbox on one of our forms, or otherwise opt in to text messaging, we use your phone number to send the categories of messages described in our SMS Terms and Conditions, for example appointment reminders, appointment confirmations, insurance-verification communication, customer care, and other service-related messages.

No mobile opt-in data or text messaging consent will be shared with third parties or affiliates for marketing or promotional purposes.

Your phone number and SMS consent record may be shared only with the service providers necessary to operate our texting platform (such as our business texting provider) and only to deliver the messages you consented to receive, never sold, and never shared with third parties or affiliates for their own marketing purposes.

If we ever intend to send promotional or marketing text messages, that consent is collected through a separate, specifically labeled marketing checkbox it is never bundled with, or implied by, consent to receive appointment or customer-care messages. 4. How Information Is Disclosed

We do not sell your personal information. We may disclose information only in the following limited circumstances:

  • Service Providers: With vendors who help us operate our Website, scheduling system, insurance-verification tools, or SMS/texting platform (including our business texting provider), solely to provide those services on our behalf and under confidentiality obligations.
  • Legal Requirements: When required to comply with applicable law, regulation, subpoena, or government request.
  • Business Transfers: In connection with a merger, acquisition, or sale of business assets, subject to standard confidentiality protections.
  • With Your Consent: In any other circumstance where you have given explicit permission.


As stated in Section 3, SMS consent and mobile opt-in information are never shared with third parties or affiliates for marketing or promotional purposes, regardless of any other disclosure described above.

5. Website Forms, Cookies, Analytics & Third-Party Integrations

Website Forms: We use forms such as Contact Us, appointment-request, insurance-verification, and new-patient inquiry forms to collect the information described in Section 1. Submitting a form does not, by itself, opt you in to text messages. SMS consent is collected only through the separate, optional checkbox described in our SMS Terms and Conditions.

Cookies & Analytics: Our Website may use cookies and analytics tools to understand site traffic and improve the user experience. You can control cookie preferences through your browser settings.

Scheduling System: Our online scheduling tool may be operated by a third-party vendor. Information entered into the scheduling system is used solely to book, confirm, and manage your appointment.

Business Texting Platform: Our business telephone number is registered with a business texting provider (Mango Voice) solely to send and receive the text messages described in our SMS Terms and Conditions.

6. Protected Health Information (PHI) & Sensitive Medical Information

Our standard website forms are not intended to collect detailed diagnoses, extensive symptom descriptions, medical records, Social Security numbers, insurance-card images, identification documents, or other sensitive medical information.

Please do not submit urgent or highly sensitive medical information through our website forms. Our forms are not intended for medical emergencies. Call 911 for an emergency.

If a specific form on our Website is used to collect protected health information (PHI), that form will separately disclose: the platform used, whether it is HIPAA-capable, whether a Business Associate Agreement is in place, how submissions are encrypted, where information is stored, who has access, and how long it is retained. Any PHI collected as part of your clinical care is otherwise governed by our HIPAA Notice of Privacy Practices, available at the clinic upon request.

7. Data Security & Retention

We implement reasonable administrative, technical, and physical safeguards designed to protect your personal information from unauthorized access, disclosure, alteration, or destruction. We retain personal information, including SMS consent records, only as long as necessary to fulfill the purposes described in this Policy or as required by law. No method of transmission over the internet or mobile network is completely secure, and we cannot guarantee absolute security.

8. Your Choices and Rights

You may:

  • Opt out of SMS communications at any time by replying STOP to any text message, or by contacting us using the information in Section 11.
  • Request access to, correction of, or deletion of your personal information.
  • Decline to provide optional information on our forms, including SMS consent, without affecting your ability to request services.

9. Children's Privacy

Our Website and texting program are not directed at children under the age of 13, and we do not knowingly collect personal information from children under 13 without parental consent.

10. Changes to This Policy

We may update this Privacy Policy from time to time. The “Last Updated” date above reflects the most recent revision. Continued use of our Website or texting program after changes are posted constitutes acceptance of the updated Policy.
